• Arrange an appropriate appointment for you.

• Ensure we are offering you the most appropriate care.

• Maintain our standards of care.

Providing some information is optional. If you choose not to enter the information requested in the health questionnaire via the online form please contact us to discuss your options. If you prefer, we can arrange to call you to collect this information over the phone.

 If you choose not to provide the information requested in the health questionnaires either online or on the phone, you will need to see your GP to ask for a referral the clinic.

We keep your information safe by storing it in secure patient management systems and using hospital based email systems.  Only staff directly related to your care are allowed to access it.  

Statistical data: Should you undertake surgery, non identifiable data is collected for the purpose of surgical safety and outcome audits.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please contact us at specialist@wakefield.co.nz
